2013 Indiana Soccer Camp Enrollment Information

Printable/Mail-in Applications will be available in early 2013. Mail in application and parental consent form along with a non-transferable deposit of $200 (balance due by April 1, 2013) or you may send the complete camp fee of $575 at once. No telephone reservations are accepted. No money orders are accepted. If applying after April 1, the total fee must accompany application. Registration remains open until session is full. Pay by Credit Card

If you prefer to mail in your credit card information, please complete the application (email us at info@indianasoccercamp.com to receive a printable form) and parental consent form with credit card information and mail or fax them to our office. The full payment is charged when using a credit card and will not be processed without the parental consent form.
Confirmation Letter/Additional Information
Upon receipt of the camper's application and parental consent form, a thorough checklist of items necessary for camp will be sent to you via email. This letter includes complete registration information. Please call (812) 219-6811 if you have not received confirmation immediately following online registration, or within 3 weeks of mailing in an application.
Refund/Cancellation Policy
If you need to cancel your camper, we must have a note of cancellation faxed to (812) 855-7150 or emailed to info@indianasoccercamp.com. For any cancellation prior to 8 days before the start of camp, all fees refunded except a $75 handling and processing fee. For cancellations occurring within the week prior to the start of camp, all fees refunded except a $150 handling and processing fee. Day of camp cancellation, no refund. Application fees and deposits are non-transferrable.
Eligibility
The camp is open to any and all participants, boys ages 10-19, pre-college. NCAA legislation allows high school graduates to attend camp.
Physicals
No physical examination is required. Each camper must have a new, completed parental consent form on file with the camp each summer. The form is on the camp application/printable brochure. Current tetanus is required. Campers should arrive at camp in good physical condition. Trainers are available at registration to discuss health conditions that may require attention during the week.
Insurance
The parent/guardian must provide primary insurance for their camper. The Indiana Soccer Camp provides only secondary accident/medical insurance.
Individual Check-in/Check-out
Campers will register between 1:00 p.m. and 3:00 p.m. on Wednesday, the first day of the session. The first training session is Wednesday at 3:30 p.m. Camp will conclude at 12 noon on Sunday following the 11:30 a.m. Awards Ceremony. Please plan to attend!
Camp Ball
Each camper must have a soccer ball at camp. Bring your own ball or purchase a custom made, hand-stitched size 5 adidas ball featuring the IU logo. The price of this ball is $30.00 if paid in advance. Availability is limited. Please check the appropriate box on the application or during online registration. A soccer ball may be purchased at camp registration for $35.00 while available. Campers will pick up their ball during registration.
